Our Complex Care Service is a bespoke, person centred service, unique to each individual. This established service is led by a team with many years’ experience working with vulnerable clients, families and multidisciplinary teams. A holistic approach is taken with all our care packages, ensuring that all our clients’ needs are met.
We provide Complex Care at Home, within Hospital/ Care Setting.
Our dedicated Health Care team is made of Rehabilitation Support workers and Qualified Nursing Staff, with other disciplines available on request.
We offer Complete clinical care management, detailed care and support plans and risk assessments.
At Greenshoot Specialised Care Services we exercise a collaborative approach with clients and their families. Our Care Coordinators visits clients regularly in order to discuss your existing care and on-going needs ensuring a person-centred approach is taken in line with your rehabilitation goals.

Acquired brain injury
We work with a number of Case Managers, Solicitors, Commissioners and Rehabilitation Units in providing rehabilitation services for adults with acquired Brain Injury. This service can be provided either at the client’s home or prior to discharge from a neurological centre to allow for a seamless transition into the community.
